Community Aspects of Chronic Disease Management Hub

Chronic diseases are b​oth very common and very costly; the establishment of a hub dedicated to this subject enables researchers from multiple disciplines to study the subject of chronic diseases from various complementary perspectives. Our initial focus is end of life care and palliative care with an emphasis on end-stage heart and renal disease, chronic pulmonary disease and advanced dementia, evaluating hospital and community involvement, continuity of care and collaboration.
